We use a service to text patients a link to complete patient forms before their appointment. This service also enables two-way texting between patients and staff, allowing us to directly assist you with any questions you may have.
Filling them out online prior to your appointment will allow us to attend to your dental needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
Payment Options
We accept payment via cash, checks, all major credit cards, and CareCredit for your convenience. Additionally, we now offer the option to text patients a link for easy online payments.
